Primecare Home Care Services is seeking a dedicated Personal Care Assistant to implement customized care plans, support clients at home, and promote quality of life. You will work under supervision to assist with daily living tasks and monitor clients' well-being.

You will treat clients like family, demonstrate empathy, and communicate effectively with clients and supervision. This role requires reliable punctuality and a compassionate approach to home-based care.

Qualifications

• Experience in home health care as a Personal Care Assistant.
• HHA or PCA Certification and valid BLS/CPR.
• TB Screening/PPD Chest X-Ray within 30 days and valid driver’s license.

Responsibilities

• Carry out assigned tasks on clients and follow Clients Care Plans.
• Help clients maintain personal hygiene and assist with activities of daily living.
• Take and record vital signs (blood pressure, temperature, respiration).
• Provide companionship to the client when needed.
• Recognize client needs, maintain good relationships, and communicate meaningful observations to supervisor.
