Job summary

Ennoble Care is seeking a Patient Care Coordinator to join our Marlton, New Jersey office. You will work with clinicians, staff and patients to ensure high-quality care and maintain open communication while managing care providers and scheduling for families.

The ideal candidate is compassionate, exceptionally organized, and capable of multitasking in a fast-paced environment. Proficiency with Outlook and MS Office is required for daily tasks.

Qualifications

• Must be able to speak on the phone for long periods
• Must be compassionate and empathetic toward patients with exceptional customer service
• Ability to take accurate notes and document tasks timely
• Ability to multitask between patients and workstreams while staying organized and efficient
• Thrives in a fast-paced environment
• On-site Marlton, NJ: Monday–Friday, 8:30am–5:00pm
• Proficient in using a computer, including Outlook and other Microsoft Office programs

Responsibilities

• Complete individualized patient care plans and perform care management and care coordination using Ennoble Care’s EMR
• Frequent contact with patients to provide care coordination and manage program compliance to improve outcomes
• Document all client communications accurately
• Communicate with Primary Care Clinician regarding patient needs and ensure care plans are complete
• Keep Team Supervisor informed of care plan issues
• Collaborate across cross-organizational teams to meet patient needs
• Become skilled at using technology including secure email, telephone system, EMR, etc.
• Adhere to documentation protocols and daily work logs
• Provide excellent customer service evidenced by positive feedback
• Support team members to contribute positively to the department
