Qualifications

Responsibilities

• Frequent contact with patients to provide support and manage care programs.

• Coordinate with Primary Care Clinician or designee regarding emergent needs.
• Work with cross-organizational teams to meet patient needs and care plans.
• Use technology including secure email and electronic medical records; maintain logs and communications.
