Job summary

Ennoble Care is seeking a full-time Patient Care Coordinator to join our Marlton, New Jersey office. You will work with clinicians, staff, and patients to coordinate high-quality care, manage caregivers, and ensure clear communication.

The role requires compassion, organization, and the ability to handle multiple workstreams in a fast-paced environment. Typical duties include developing care plans, maintaining EMR documentation, and supporting care teams.

Qualifications

• Comfortable speaking on the phone for large portions of the day.
• Compassionate and empathetic toward patients with excellent customer service.
• Ability to take accurate notes and document tasks promptly.
• Able to multitask between patients and workstreams while staying organized.

Responsibilities

• Create individualized patient care plans and perform care management using the EMR.
• Maintain frequent contact with patients to provide care coordination and ensure program compliance.
• Document all client communications accurately (verbal or written).
• Coordinate with Primary Care Clinician or designee regarding patient needs and care plans.
• Inform Team Supervisor of issues related to the care plan process and potential risks.
• Collaborate with cross-functional teams to meet patient needs and family members' expectations.
• Become proficient with technology including secure email, phone systems, and EMR.
• Adhere to documentation protocols and daily work logs; escalate issues as needed.
