Qualifications

Responsibilities

• Coordinate the new patient intake process from referral through scheduling.
• Communicate with patients, families, senior living communities, and referral sources.
• Schedule and confirm patient appointments and collect demographic/insurance data.
• Verify intake information and ensure all forms are completed accurately.
• Maintain accurate patient records in EHR/tracking systems.
