Job summary

Qualifications

Responsibilities

• Identify patient/family/facility care needs and provide nursing and supportive care for the terminally ill and their families.
• Report to Team Manager and coordinate care within Hospice/Home Care setting.
• Maintain infection control standards and accurate EMR documentation.
