Job summary

GBMC HealthCare is seeking a Hospice Home Care Nurse to join the team. The nurse identifies home care needs of patients and families, develops and executes care plans, and provides direct nursing care in alignment with the physician's orders.

The role requires an RN license in Maryland, a nursing degree (Diploma/ADN/BSN), and at least two years of nursing experience. Strong communication and EMR skills are essential for coordinating with care teams and educating families.

Qualifications

• Nursing degree: Diploma, ADN or BSN from an accredited program.
• Current Maryland RN license.
• Two years of nursing experience or equivalent education/experience.
• Strong infection control knowledge and self-protection practices.

Responsibilities

• Perform initial patient and family assessment to determine home care needs.
• Develop and implement the care plan with the Interdisciplinary Group.
• Provide direct nursing care in coordination with the primary physician.
• Plan and supervise home health aide delivery of care.
• Communicate updates to physician and interdisciplinary team; attend care conferences.
• Document in facility chart and reconcile medications with staff.
• Educate family and facility staff on patient care tasks and safety.
