Job summary

GBMC HealthCare is seeking a Hospice Home Care Nurse to identify patient and family care needs and provide nursing and supportive care for terminally ill patients and their families in home or facility settings.

The role requires a registered nurse with a current Maryland license, 2 years of nursing experience, and ability to work with an interdisciplinary team to create and implement care plans that emphasize comfort and family involvement.

Qualifications

• Diploma, ADN and/or BSN from an accredited institution.
• Current Maryland State license as a Registered Nurse.
• Two years of nursing experience, or equivalent combination of education and experience.

Responsibilities

• Completes initial assessment of patient and family to ascertain home care needs.
• Prepares and implements the care plan for patient/family unit in collaboration with the Interdisciplinary Group.
• Provides direct nursing care to patients in order to maintain highest level of care.
• Plans and supervises the delivery of care by the HHA.
• Uses problem oriented charting to ensure continuity of care in a home setting.
