GBMC HealthCare in Towson, MD is seeking a Hospice RN Weekender to identify patient and family care needs and provide nursing and supportive care for terminally ill patients and loved ones. The role reports to the Team Manager and works Saturdays and Sundays 8am–8pm.

Requirements include Maryland RN license, 1–2 years nursing experience, and education: Diploma, ADN or BSN. Hospice or Home Health experience preferred. Proficiency with EMR and Blackberry is desirable.

Qualifications

• Maryland RN license current.
• 1–2 years nursing experience; hospice/home health preferred.
• Diploma/ADN/BSN from an accredited institution.

Responsabilités

• Complete initial assessment of patient and family to determine home care needs.
• Prepare and implement the care plan with the Interdisciplinary Group.
• Provide direct nursing care and coordinate with physicians.
• Educate family or caregivers on care skills and medication administration.
• Arrange home equipment and address patient/family needs at the time of death.
