Job summary

OneWell Health Care—Philadelphia field-based DSP position—is seeking a dedicated caregiver to provide hands-on support to individuals with IDD in home and community settings. The DSP will implement ISP/IPP plans, assist with daily living, transportation, and community participation, and document care activities with HIPAA-compliant records.

The role emphasizes independence, dignity, and person-centered care, with opportunities for training and collaboration with an interdisciplinary team.

Qualifications

• High school diploma or equivalent required.
• Experience in caregiving, home health, or direct support for IDD.
• Ability to use EVV systems and electronic documentation.
• Ability to follow written care plans and documentation guidelines.

Responsibilities

• Provide hands-on, person-centered support according to each care plan.
• Assist with daily living activities, meals, mobility, and medications as authorized.
• Implement behavior support strategies and monitor health, safety, and well-being.
• Maintain accurate, timely documentation and participate in trainings and team meetings.
