Job summary

OneWell Health Care is seeking a dedicated Direct Support Professional to provide hands-on, person-centered care for individuals with IDD in home and community settings. You will implement ISP/IPP plans, support daily living activities, and promote independence and dignity.

Candidates should have a high school diploma or equivalent, experience in caregiving preferred, and the ability to use EVV systems and electronic documentation.

Qualifications

• Experience in caregiving, home health, or direct support preferred.
• Ability to use electronic documentation and EVV systems.

Responsibilities

• Provide hands-on, person-centered support per ISP/IPP.
• Assist with personal care, daily living activities, meals, mobility, and medications as authorized.
• Maintain accurate documentation and participate in trainings and team meetings.
