Job summary

OneWell Health Care seeks a dedicated Direct Support Professional (DSP) to provide hands-on, person-centered care to individuals with IDD in home and community settings in Flourtown, PA. This field-based role involves supporting daily living activities, behavior plans, and community participation while ensuring health, safety and dignity.

The DSP will follow ISP/IPP guidance, document accurately, and participate in trainings. A high school diploma is required; bilingual skills are a plus.

Qualifications

• High school diploma or equivalent required.
• Ability to follow written care plans and documentation guidelines.
• English proficiency required; bilingual is a plus.

Responsibilities

• Provide hands-on, person-centered support to individuals with IDD in home and community settings.
• Assist with daily living activities, mobility, nutrition, and medication assistance as authorized.
• Maintain accurate documentation and participate in trainings and team meetings.
• Support transportation to appointments and community outings.
