Whole Life seeks compassionate Direct Support Professionals in Brooklyn, CT to provide person-centered care for individuals with developmental and/or physical disabilities. Qualifications

• Proof of High School diploma/ GED or equivalent.
• Proof of valid driver’s license, SSN, birth certificate or passport.
• Ability to pass a Background Check, safe driving record, and Abuse Registry Check; post-hire Fit for Duty and TB test within 30 days of hire.
• Must be able to stand, move, twist, bend, kneel, reach, lift and walk; some positions up to 100 lbs.
• Must document care in English and communicate with clients/co-workers verbally and in writing.

Responsibilities

• Assist with Activities of Daily Living (ADLs): bathing, dressing, grooming, toileting, and meal preparation.
• Administer medications and monitor vital signs; support medical appointments.
• Maintain a clean, safe living environment through light housekeeping and laundry.
• Engage clients in social activities and community outings to promote independence.
• Use mobility equipment (e.g., Hoyer lifts) with proper techniques.
• Document daily activities, observations, and incidents accurately.
• Maintain confidentiality per HIPAA regulations.
• Implement individualized care plans.
