Job summary

Whole Life is seeking compassionate Direct Support Professionals to join our team in Brooklyn, CT. This role focuses on individualized, person-centered care for adults with developmental and intellectual disabilities, supporting daily living activities, promoting independence, and encouraging community integration.

You will work closely with families and professionals to implement care plans, document progress, and foster clients' safety and dignity in daily routines.

Qualifications

• Proof of High School diploma, transcripts, GED equivalent.
• Proof of valid driver's license, social security card, birth certificate or passport.
• Ability to pass background check, safe driving record, abuse registry check; medical clearance after hire.

Responsibilities

• Assist with Activities of Daily Living (ADLs) and meal preparation.
• Administer medications, monitor vital signs, attend medical appointments.
• Maintain a clean, safe living environment through light housekeeping and laundry.
• Engage clients in social activities and community outings to promote independence.
• Use mobility equipment with proper techniques.
• Document daily activities, observations, and incidents accurately.
• Maintain confidentiality per HIPAA regulations.
