Job summary

Whole Life seeks compassionate Direct Support Professionals to join our team in Brooklyn, CT. DSPs provide person-centered care to adults with developmental and intellectual disabilities, supporting daily living activities and promoting independence and community involvement.

Responsibilities include ADL assistance, medication support, documentation, and collaboration with families and managers to ensure safety, dignity, and well-being.

Qualifications

• Proof of High School diploma/ Transcripts/ GED equivalent.
• Proof of valid driver's license, social security card, birth certificate, or passport.
• Ability to pass a Background Check and health screenings, including a TB test within 30 days.
• Must be able to stand, move, lift, and perform physical tasks, with occasional lifting up to 100 lbs.
• Must document care in English and communicate in English with clients and co-workers.

Responsibilities

• Assist with Activities of Daily Living (ADLs): bathing, dressing, grooming, toileting, and meal preparation.
• Administer medications, monitor vital signs, and support medical appointments.
• Ensure a clean, safe living environment through light housekeeping and laundry.
• Engage clients in social activities, community outings, and choice-making to promote independence.
• Use mobility equipment with proper techniques.
• Document daily activities, observations, and incidents accurately.
• Maintain confidentiality per HIPAA regulations.
• Implement individualized care plans.
