Qualifications

• Ability to physically assist individuals as required and intervene safely during crises.
• Ability to evacuate a residence in emergencies following procedures.
• Ability to communicate effectively with individuals served, families, and staff.

Responsibilities

• Participate in development and implementation of each person’s plan of support.
• Record documentation in electronic health records or equivalent systems.
• Assist and supervise individuals in daily activities and community involvement.
• Provide health, hygiene, and mobility support as needed.
• Transport individuals to appointments and activities.
• Assist in crisis intervention and safety procedures.
• Support home maintenance and meal preparation as required.
