Job summary

New Horizons Resources Inc. in Wallkill, NY is seeking a Direct Support Professional for a 2nd shift role. You will assist individuals with safety, life-skills training, and community participation under Life Plan guidelines.

The DSP works with the residence team, coordinators, clinicians, and families to support daily living, behavior support, transportation, and inclusive activities in the community. Shift: Mon-Fri 3pm-11pm.

Qualifications

• High school diploma or equivalent.
• Ability to physically assist and intervene safely.
• Able to evacuate in emergencies per guidelines.
• Good communication with individuals with DD.
• Valid driver license and ability to drive agency vehicles.
• Experience with people with disabilities.
• Willingness to complete training within six months.

Responsibilities

• Develop and implement life plans and documentation.
• Record daily progress in electronic health record.
• Assist individuals with activities, budgeting, and daily living.
• Transport to medical appointments and community activities.
• Intervene in behavioral crises using SCIP/Ukeru as indicated.
• Support Residence Coordinator and team.
