Job summary

WellCare Homes is seeking a Direct Support Professional to provide direct care, supervision, and support to adults with developmental disabilities in a licensed residential setting. You will assist with daily living activities, manage documentation, and participate in team-based care planning to promote independence and safety.

The role requires lifting and assisting with mobility, adherence to Title 17/22 regulations, and completion of trainings.

Qualifications

• High School Diploma or GED required.
• Valid and current California driver’s license with acceptable driving record.

Responsibilities

• Provide direct care and supervision to adults with developmental disabilities, including activities of daily living.
• Support clients in achieving individualized goals identified in IPP/IBSP/Health Care Plan.
• Observe and report changes in physical condition, behavior, and safety concerns.
• Maintain accurate documentation and medication logs in compliance with laws.
