Job summary

OneWell Health Care in Pennsylvania is seeking a Direct Support Professional (DSP) to provide hands-on, person-centered support to individuals with IDD in home and community-based settings. This role emphasizes daily living activities, skill development, health and safety monitoring, and promoting independence and community inclusion.

Qualifications

• High school diploma or equivalent.
• Preferred: caregiving, home health, or direct support for individuals with IDD.
• Experience in community-based or residential support settings is a plus.

Responsibilities

• Provide hands-on, person-centered support to individuals with IDD in home and community-based settings.
• Assist with personal care and daily living activities, meal preparation, mobility, and medication assistance as authorized.
• Implement behavior support strategies and monitor health, safety, and emotional well-being.
• Promote independence, stability, and community participation by maintaining structured routines and supporting employment or vocational activities.
• Assist with transportation to appointments or community outings and provide household support related to care needs.
• Participate in trainings and team meetings and maintain accurate documentation.
