Qualifications

Responsibilities

• Ensure client's health, wellness and safety.
• Encourage goals to learn independent living skills.
• Crisis prevention and intervention.
• Promote advocacy and individual rights.
• Maintain detailed, compliant documentation of client status and behaviors.
• Apply behavior support plan strategies for the individual served.
• Report suspected abuse, neglect, and exploitation promptly.
• Use prompting, instruction, modeling and reinforcement for person-centered services.
• Promote independence in activities of daily living and community activities.
• Provide transportation as needed for client.
