Job summary

PACE SEMI seeks a Community Living Associate (CLA) to provide supportive services to participants in the community setting, during transport, and in the day health center under supervision. duties include personal care, meals, homemaking, and coordination of care per the participant care plan.

Required a high school diploma or GED, a Michigan driver's license, CPR/BLS, and at least one year of experience with frail or elderly populations. CNA or Direct Care cert preferred.

Qualifications

• HS diploma or GED required with direct care experience.
• 1 year of experience with frail or elderly population preferred.
• Certification (Nurse/Direct Care) outside of PACE SEMI is acceptable.

Responsibilities

• Provide personal care up to 2-person assist as outlined in the care plan.
• Offer 1:1 interaction in DHC and CL per plan of care.
• Assist with activities and meal preparation or feeding as needed.
• Assist with toileting and incontinence management.
• Coordinate care with the DHC based on the care plan and communicate progress.
