Job summary

PACE Southeast Michigan is seeking a Community Living Associate (CLA) to provide supportive services to residents in community settings, during transport, and at the day health center. The CLA collaborates with the manager and care team to implement each participant's care plan and to promote independence and well‑being.

Key duties include assisting with activities of daily living, meals, mobility, and personal care, while ensuring accurate documentation and timely reporting of issues.

Qualifications

• H.S. diploma or GED required.
• Valid Michigan Driver's license.
• Current CPR card required.
• Certification optional but preferred, but must have one year experience with a frail or elderly population.

Responsibilities

• Provide supportive services in the community setting, during transport, and at appointments or in the day health center under supervision.
• Follow the participant care plan and assist with activities of daily living as outlined.
• Maintain accurate timely documentation of services provided and progress.
• Report deviations from schedules and any customer service issues to supervisor.
• Assist with meals, toileting, personal care, and housekeeping as needed.
