Job summary

PACE Southeast Michigan seeks a Community Living Associate (CLA) to provide supportive services to residents in community settings and during transport and appointments, under supervision of the manager, RNCM, or lead CLA.

The CLA delivers care per individual care plans, assists with activities of daily living, meals, and homemaking, and maintains accurate documentation while coordinating with the clinical team.

Qualifications

  • Requires a high school diploma or GED.
  • Valid Michigan driver's license and up-to-date immunizations.
  • Current CPR card required; CNA training preferred.
  • Completion of basic nursing assistant training; certification outside PACE SEMI preferred.
  • Annual in-service trainings including infection control, TB testing, safety, and BLS.

Responsibilities

  • Provide personal care and 1:1 interaction as outlined in the care plan.
  • Assist with activities, meals, toileting, and nutritional needs.
  • Perform homemaking, laundry, shopping, and coordination of care.
  • Document accurately and timely; inform supervisor of progress and issues.
  • Respond to emergencies per policy; maintain scope of practice.
  • Drive within catchment area; ensure reliable transportation.
