Job summary

PACS Group is seeking a dedicated nursing assistant to provide routine daily care and services to residents in accordance with assessments and care plans. You will work under supervision to perform a wide range of duties from daily living activities to reporting changes in residents' conditions.

Responsibilities include assisting with meals, mobility, tests preparation, documentation, and maintaining a safe, caring environment on the unit.

• Identify residents before administering care or meals using wristband/photo file.
• Record information using only facility-approved abbreviations.
• Report changes in the resident's condition to the Nurse Supervisor promptly.
• Document entries on flow sheets, notes and charts in a clear, descriptive manner.
• Report any accidents or incidents observed during the shift.
• Protect resident privacy; report suspected unauthorized disclosures to the Administrator.
• Report any unauthorized attempts to access the facility's information system.
• Perform only nursing tasks you are trained to do.
• Ensure the resident's room is ready for arrival (bed made, tags, kit available).
• Greet residents and escort them to their room.
• Introduce resident to roommates and staff as appropriate.
• Make residents comfortable and at ease.
• Inventory and label residents' personal possessions.
• Store residents' clothing securely.
• Assist with packing when transferring or discharging residents.
• Transport residents to new rooms or to the receiving area.
• Assist with loading/unloading residents to/from vehicles as needed.
• Create a warm, personal, calm atmosphere on the unit and shift.
• Meet with nursing staff to identify problems and improve services.
• Report all resident complaints and grievances.
• Participate in and receive the nursing report as instructed.
• Follow policies on exposure to blood and body fluids.
• Make beds and add extra covers as requested.
• Measure and record temperatures, pulses and respirations; weigh and measure when instructed.
• Check residents frequently if they need assistance calling for help.
• Answer resident calls promptly and check daily care needs.
• Assist residents with food choices and tray placement when needed.
• Record food/fluid intake and report changes in eating habits.
• Keep water pitchers clean and filled and within reach.
• Perform after-meal care including cleaning hands and face.
• Participate in the nursing report at the start of each shift.
• Follow established policies and procedures and supervisor instructions.
• Cooperate with other departments to maintain nursing services.
• Notify the facility if you will be late or absent.
• Report occupational exposures to blood, body fluids, infectious materials and chemicals.
• Assist with daily self-care tasks (dental care, bathing, grooming, dressing).
• Keep residents dry; replace wet clothing or linens promptly.
• Assist with preparing residents for medical tests (labs, x-ray, therapy, etc.).
• Assist with lifting, turning, moving, positioning, and transporting residents.
• Perform restorative and rehabilitative procedures as instructed.
• Aid in preparing residents for physical examinations.
• Check residents routinely to ensure personal care needs are met per their wishes.
• Observe skin integrity and report signs of pressure injuries early.
• Report injuries of unknown origin and any skin tears.
• Observe disoriented and comatose residents and record data as instructed.
• Perform any other special treatments as instructed.
