Job summary

PACS Group is seeking a dedicated Certified Nursing Assistant to provide routine daily nursing care according to care plans. You will identify residents, document care, report changes, and assist with daily functions in a calm, safe environment.

The role requires adherence to privacy rules, accurate record-keeping, and teamwork with nursing staff to ensure high-quality resident care. This is an on-site position in Texas, with standard shift duties and emergency response responsibilities.

Qualifications

• Must possess, as a minimum, an 10th grade education.
• Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations.
• Ability to write reports, business correspondence, and procedure manuals.
• Ability to effectively present information and respond to questions from managers and employees.
• Mathematical Skills: apply concepts such as fractions, percentages, ratios and proportions to practical situations.
• Reasoning Ability: solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists.

Responsibilities

• Identify residents using the wristband or photo card before administering treatments or during meals.
• Record entries on flow sheets, notes, charts and other documents clearly and descriptively.
• Report all accidents, incidents, or changes in resident condition to the Nurse Supervisor/Charge Nurse.
• Maintain confidentiality of resident care information, including protected health information.
• Assist residents with daily functions and mobility, including dressing, grooming, bathing, and transferring.
• Observe for pressure injuries and skin breakdown and report promptly.
• Participate in nursing reports and follow facility policies and procedures.
• Assist with evacuations during emergencies as needed.
• Ensure rooms and equipment are prepared for residents and ensure a calm environment.
