Job summary

Rocky Mountain Care-Clearfield in Clearfield, UT, is seeking a Certified Nursing Assistant (CNA) to provide compassionate care to residents under the supervision of nursing leadership. This is a full-time, day shift (6a–6p) position inside a skilled nursing/long-term care setting.

The CNA will perform activities of daily living, assist with mobility, provide feeding assistance, monitor resident condition, and document care in accordance with care plans and policies.

Qualifications

• Certified Nursing Assistant in good standing with the state.
• High school diploma or equivalent.
• Current CPR/BLS certification.
• Current Food Handler Permit.
• Must have no disciplinary action in effect against certification.
• At least one year of experience as a CNA in long-term care (preferred, not required).

Responsibilities

• Provides supportive services to nurse, med tech and other staff as needed and performs duties as assigned.
• Assists resident with activities of daily living per care plans and policies.
• Assists with lifting, turning, moving, positioning, and transporting residents.
• Coordinates dining room services, meal tray delivery, feeding assistance and documentation.
• Delivers nutritional supplements and documents intake.
• Records resident weight and reports changes to nurse.
• Monitors skin condition and reports issues to nurse.
• Assists with toileting programs and completes flow sheets.
• Keeps supervisor informed of factors affecting performance.
• Performs CNA duties as staffing needs require and adheres to safety guidelines.
