Responsibilities

• Provide direct and indirect resident care under the direction of a licensed nurse.
• Follow facility policies and procedures for safety, privacy, and dignity.
• Assist with admission and discharge of residents.
• Assist with daily living activities: oral care, bathing, hair, nails, and dressing.
• Assist with mobility, transfers using gait belts, and positioning.
• Take and document temperature, weight, and vital signs as directed.
• Assist with meals, tray setup, feeding, and hydration.
• Support activity participation and transportation to appointments or programs.
• Report changes in resident condition to the Charge Nurse.
• Prepare residents for activities and maintain clean, organized rooms.
