Job summary

Evergreen Post Acute in Portland, Oregon seeks a dedicated Certified Nursing Assistant to provide routine daily nursing care and services to residents per care plans. You will identify residents, assist with activities of daily living, document care and respond to calls, under supervision.

Training is provided; shift-based responsibilities include patient comfort, safety, and reporting changes to the Nurse Supervisor.

Qualifications

• Must be a licensed Certified Nursing Assistant.
• Able to read, write, and communicate effectively in English.
• Ability to follow care plans and report changes to supervisors.
• Physical ability to lift up to 25 pounds and assist residents.

Responsibilities

• Identify residents before treatments, meals, etc., using wristband/photo card.
• Record all entries on flow sheets, notes, charts, etc., clearly.
• Report changes in resident condition to Nurse Supervisor/Charge Nurse promptly.
• Assist residents with daily activities and mobility, maintaining a warm, safe environment.
