Job summary

Woodside Post Acute in Molalla, Oregon seeks a compassionate Certified Nursing Assistant to provide routine daily nursing care to residents per their care plans and supervisor instructions.

You will identify residents using wristband/photo ID before procedures, assist with activities of daily living and mobility, document care accurately, observe changes, and collaborate with the nursing team to maintain safety, comfort, and respectful treatment at all times.

Qualifications

• Must possess a licensed Certified Nursing Assistant in accordance with Oregon state laws.
• CPR certification preferred.
• Minimum education: 10th grade or equivalent.
• Ability to read, write, and follow written procedures.

Responsibilities

• Provide routine daily nursing care per resident assessment and care plan.
• Identify residents using wristband/photo ID before procedures.
• Assist with activities of daily living and mobility as needed.
• Record entries accurately in flow sheets and charts.
• Report changes in condition to Nurse Supervisor/Charge Nurse promptly.
• Maintain confidentiality of resident information and safety protocols.
• Prepare resident rooms, greet and orient residents, and assist with transfers.
• Observe for skin integrity and document findings; report concerns.
• Collaborate with nursing staff to ensure safe and respectful care.
