Responsibilities

• Identify residents before treatments or meals using wristband or photo ID.
• Record entries on flow sheets, notes and charts in an informative manner.
• Report changes in residents' condition to Nurse Supervisor/Charge Nurse promptly.
• Maintain confidentiality of resident care information and report unauthorized disclosures.
• Escort residents to rooms and ensure comfort and safety during transfers.
• Assist with daily living activities, including eating, hygiene, mobility, and positioning.
• Observe for pressure areas and skin breakdowns and report findings.
• Follow established policies and procedures and cooperate with facility staff.
