Job summary

FOREST GROVE POST ACUTE is seeking a Certified Nursing Assistant (Healthcare) to provide routine daily nursing care to residents in accordance with care plans. This PRN role supports the nursing team in delivering compassionate, high-quality care in a patient-centered environment.

Responsibilities include verifying resident identity, accurately recording observations, assisting with daily activities, reporting changes to the Nurse Supervisor, and maintaining resident dignity and safety.

Qualifications

• Must possess, as a minimum, a 10th grade education.

Responsibilities

• Provide routine daily nursing care to residents per their care plans and as directed by supervisors.
• Identify residents before administering treatments using wristband or photo card.
• Record entries accurately on flow sheets, notes, and charts.
• Report changes in a resident's condition to the Nurse Supervisor/Charge Nurse as soon as practical.
• Maintain confidentiality of resident information and report any suspected disclosures.
