Job summary

Qualifications

• Must meet all applicable state and federal requirements for this position.
• Prefer 6 months or more experience in home care, assisted living, or long-term care facility.

Responsibilities

• Assist residents with activities of daily living (ADL) tasks.
• Assist with moving and transferring residents and ensure safety.
• Communicate incident reports and changes in resident condition to a supervisor.
• Provide medication assistance following provider instructions and documentation.
• Maintain accurate documentation of care and medication administration.
• Support residents to participate in activities and outings as needed.
• Follow infection control practices and safety procedures.
