Job summary

PACS is seeking a dedicated Nursing Assistant to provide routine daily care to residents according to assessments and care plans, under supervision. You will greet residents, assist with meals, personal care, mobility, and documenting care activities.

Responsibilities include reporting changes to supervisors, maintaining privacy, handling incident reports, and cooperating with nursing staff to ensure a safe, comfortable environment.

Qualifications

• Must possess a high school education or its equivalent.

Responsibilities

• Provide routine daily care per resident assessment and care plan.
• Use PCC to ensure correct resident information when assisting with ADLs and meals.
• Report changes in residents’ condition to Health and Wellness Director promptly.
• Document all entries clearly in notes, SPAs, and incident reports.
• Maintain confidentiality of resident information and report any unauthorized disclosures.
• Assist residents with daily functions such as dental care, bathing, dressing, grooming, and mobility.
• Escort residents to rooms, prepare rooms, and ensure residents are comfortable.
