Job summary

PACS seeks a caring caregiver to provide routine daily care and support to residents per individualized care plans. You will document observations, use PCC for resident information, and promptly report any changes to supervisors.

The role includes assisting with meals, transfers, and comforting, while maintaining a safe, respectful environment. Work requires attention to detail, the ability to follow procedures, and compassionate interaction with residents.

Qualifications

• Must possess a high school education or its equivalent.
• Proficiency in Microsoft Office Suite.
• Ability to read, analyze, and interpret general business materials.

Responsibilities

• Provide routine daily care and services per care plan.
• Use PCC to verify resident information.
• Report changes to Health and Wellness Director promptly.
• Record entries clearly in notes, SPAs and incident reports.
• Escort residents to rooms and greet them warmly.
• Maintain residents' room readiness and belongings.
• Assist with transfers, meals, hygiene, and daily activities.
• Observe and report health changes and safety concerns.
