Qualifications

• Ability to collaborate with a care team and families with empathy.

Responsibilities

• Provide compassionate, respectful assistance with activities of daily living according to each resident's service plan.
• Get to know residents and tailor care to individual needs, preferences, and dignity.
• Observe and communicate changes in residents' conditions to the care team.
• Maintain documentation and follow care, safety, HIPAA, and regulatory requirements.
• Work closely with Care Partners, clinical leaders, families, and community team members.
• Assist with meals, serving, and maintaining a welcoming dining area.
