Job summary

Qualifications

• BSN or higher preferred.
• Minimum 2 years clinical nursing experience.

Responsibilities

• Coordinate interdisciplinary, patient-centered care across the continuum.

• Ensure smooth care transitions with planned handoffs.
• Partner with patients to identify issues and barriers to self-care.
• Apply health literacy guidelines to educate patients.
• Use rapid quality improvement cycles to track progress.
• Pace care coordination to balance quality and fiscal concerns.
