Responsibilities

• Evaluate and assess patient health during home visits.
• Create and update individualized care plans with physicians.
• Administer prescribed medications and monitor responses.
• Educate patients and families on disease prevention and health promotion.
• Maintain detailed health records and progress notes.
• Coordinate with providers to align care with health goals.
• Manage a caseload with prioritization by urgency.
• Respond to emergencies and take decisive action.
• Travel to patient homes as required.
