Job summary

Assured Hospice in Portland, OR, part of the LHC Group and Optum family, is seeking a Registered Nurse Case Manager to deliver the Plan of Care for hospice patients. You will collaborate with the IDG, implement treatment plans, and ensure compassionate, quality care.

Qualifications

• Current and unrestricted RN licensure in the state of practice.
• Current CPR certification or ability to obtain within 90 days of hire.

Responsibilities

• Makes the initial nursing evaluation to determine hospice eligibility within 48 hours of referral.
• Identifies patient/family needs and reassesses at least every 15 days.
• Initiates and coordinates the plan of care with the care team.
• Documents problems, goals, interventions, and patient/family responses to hospice care.
• Collaborates with patient/family, attending physician and IDG members in daily care.
• Instructs and supervises patient/family in self-care when appropriate.
