Qualifications

Responsibilities
Completes initial assessment of the patient and family to determine home care needs.
Initiates the plan of care and makes necessary revisions as patient status changes.
Regularly re-evaluates patients’ needs.
Uses health assessment data to determine nursing diagnosis.
Develops care plan establishing goals based on nursing diagnosis.
Administers medications and treatments as prescribed by the physician.
Provides health care instructions to the patient as appropriate.
