Qualifications

• Current and valid state RN license required.
• Minimum of 1 year recent acute care experience in an institutional setting.
• Basic Life Support (BLS) with hands-on CPR certification is mandatory.
• Valid driver's license and auto insurance needed.

Responsabilités

• Complete initial assessment of patient and family for home care needs.
• Initiate and revise plan of care based on patient status.
• Regularly re-evaluate patients’ needs.
• Administer medications and treatments as prescribed.
• Counsel patients and families on care plan.
