Job summary

Kin On seeks a Social Service Coordinator to oversee and implement community-based social services. You will advocate for clients, coordinate outreach, and conduct workshops addressing mental health, dementia, and caregiving needs.

You will gather client information, assess needs, and develop care plans while maintaining records and ensuring HIPAA compliance. Collaboration with departments and partners is essential.

Qualifications

• Bachelor's degree in social work; master's preferred.
• Two years of work experience in social or health services.
• Bilingual in English and Chinese preferred.
• Candidate must be authorized to work in the United States.

Responsibilities

• Support Kin On mission, vision and core values.
• Coordinate and participate in community outreach educating on mental health, Alzheimer's, dementia, Medicare/Medicaid, and long-term care services.
• Conduct workshops and presentations on mental health, caregiving, self-care, chronic disease self-management, fall prevention.
• Provide information, assistance, referral, service coordination and consultation to clients.
• Consult with family caregivers to support their caregiving needs.
• Interview and assess clients to determine needs via in-person, phone, or virtual visits.
• Develop and implement care plans per program requirements.
• Maintain client records per best practices, legality and program rules.
• Ensure operations comply with applicable rules; maintain HIPAA confidentiality.
• Communicate with regulatory and funding entities (e.g., DSHS, Aging and Disability Services, King County).
• Participate in meetings and professional development.
• Monitor and report service contracts; prepare program reports.
• Collaborate with Kin On departments and partner agencies to provide services.
• Perform other duties as assigned by Director of Community Wellness and Social Services.
