Job summary

CN Guidance and Counseling Services, Inc. seeks a Senior Care Coordinator to work within an interdisciplinary team to coordinate care for adults with severe mental illness and co-morbid conditions.

The role includes advocacy, linkage to services, and evidence-based psychosocial interventions to improve wellness and reduce hospitalizations. The position requires availability 24/7 on an as-needed basis, with travel across Nassau, Suffolk and surrounding counties as needed to support clients and

Qualifications

• Bachelor's or Master's degree in Social Work, Psychology, Education, Rehabilitation or related field preferred.
• 2 years direct service experience for BA level; 1 year for MA level in mental health or related fields.
• Proficiency in computer software and EHR systems; strong written and verbal skills.

Responsibilities

• Assess clients and document strengths, needs, goals and resources.
• Develop and update client-centered care plans with multidisciplinary team.
• Maintain clear communication with clients, physicians and providers.
