Job summary

CN Guidance & Counseling Services, Inc. is seeking a Care Coordinator to join an interdisciplinary team providing care coordination to adults with severe mental illness and co-occurring conditions.

The role advocates for clients, links to community services, and focuses on reducing emergency visits while improving wellness and self-management. The position requires flexibility to be available 24/7, with strong knowledge of care planning, documentation, and client-centered approaches.

Qualifications

• Bachelor's or master's degree in social/health fields preferred.
• Experience with adults with severe mental illness and co-morbidities.
• Bilingual preferred; strong interpersonal skills.

Responsibilities

• Assess clients to document strengths, needs and resources.
• Develop and document individualized care plans with the team.
• Coordinate care with providers to ensure goals and transitions.
• Provide outreach and linkage to community services.
• Deliver crisis intervention 24/7 as needed.
