Job summary

Kin On Health Care Center is seeking a Social Service Coordinator to oversee community-based social services and programs in the Seattle area. The role involves coordinating outreach, conducting workshops, and providing information, referrals, and care coordination to clients and families.

The coordinator will ensure HIPAA compliance, maintain client records, and collaborate across Kin On departments and partner agencies.

Qualifications

• Bachelor's degree in social work, human services or related fields is required; master's preferred.
• Two years of work experience in social or health services.
• Bilingual in English and Chinese preferred.

Responsibilities

• Oversee, coordinate and implement community-based social services and programs.
• Coordinate community outreach to educate on emotional health, dementia, Medicare/Medicaid, and long-term care.
• Conduct workshops on mental health, caregiving, self-care, disease self-management, and fall prevention.
