Job summary

Clear Destination Inc. is seeking a Social Services Coordinator to plan, organize, develop, and deliver essential social services for members and their families or caregivers.

You will manage social service assessments, summaries, and case management to address psychosocial and socio-economic needs in compliance with HIPAA and state laws. You will work with the Center Director and clinical team, coordinating with internal and external services, coordinating member plans of care, and supporting

Qualifications

• Active licensure/certification required by state board.
• Experience in health care social work preferred, especially with seniors or disabled.
• Ability to navigate HIPAA, regulatory requirements and documentation.

Responsibilities

• Coordinate social services and case management for members and families.
• Collaborate with Nurse Manager and clinical team to develop care plans.
• Lead or participate in Safety and Quality Committees and Caregiver groups.
• Evaluate referrals and determine appropriateness for admission or continued participation.
• Assist members with financial resources and access to services.
• Ensure compliance with company policies and regulatory authorities.
• Maintain accurate files and records and provide referral information as needed.
• Engage in community outreach and marketing activities.
• Provide direct support to members in activities of daily living as needed.
