Job summary

Old Colony Elder Services (OCES) is seeking a Community Transition Liaison Supervisor to provide oversight of CTLP activities and supervise the CTLP team across the south shore service area. The role ensures accurate documentation and compliance with AGE program requirements, coordinating discharge planning with facilities and CTLP staff.

Qualifications

• Bachelor's degree in social work, nursing, human services or related field.
• Current Social Work or RN license.
• Three years of supervisory/managerial experience with at least one year in transition support.

Responsibilities

• Serve as the primary CTLP contact to AGE.
• Ensure CTLP administration meets AGE programmatic requirements.
• Provide supervision and oversight to CTLP teams.
• Ensure timeliness and quality CTLP documentation in A&D.
• Monitor case records for regulatory compliance.
• Collaborate with facility staff to plan discharge activities.
• Maintain knowledge of OCES policies and funding regulations.
• Other duties as assigned.
