You will serve as primary contact for AGE and partners, provide oversight of CTLP nursing facility transition activities, ensure weekly facility visits, discharge planning participation, and timely documentation and referrals to support successful community

Responsibilities

• Direct supervision of CTLP staff
• Oversee nursing facility transition activities
• Coordinate discharge planning and community-based services
• Ensure timely documentation and referrals
• Collaborate with Fiscal Department on CTLP expenses and reporting
• Review service plans and eligibility
• Assist in hiring decisions and staff development
• Ensure timely updates in consumer files and computer system
• Conduct field visits to observe performance
• Maintain open guidance and support for CTLP staff
