Qualifications

• Bachelor's degree in social work, nursing, human services or related field required.
• Current Social Work or Registered Nurse license required.
• Three years of supervisory or managerial experience with at least one year in transition support.
• Excellent verbal and written communication skills.
• Strong interpersonal and conflict resolution skills.
• Excellent organizational skills and attention to detail.

Responsibilities

• Serves as the primary CTLP contact to EOEA staff.
• Ensures quality administration of CTLP per EOEA requirements.
• Oversees the CTLP team(s).
• Ensures timely CTLP documentation and data in Aging & Disability (A&D).
• Monitors case records for regulatory compliance.
• Develops procedures, standards, and guidelines for CTLP operations.
• Collaborates with facility, CTLP, and other OCES staff to plan discharge activities.
• Maintains knowledge of OCES policies and funding source regulations.
• Other duties as assigned.
