Job summary

OCES is expanding its Community Transition Liaison program and seeking a Supervisor to oversee CTLP activities. You will supervise CTLP teams, ensure timely documentation, and monitor case records while coordinating with facility staff to plan discharge activities.

The ideal candidate holds a social work or nursing license, a Bachelor’s degree, and 3+ years of supervisory experience with transition support expertise; strong communication and leadership skills are required.

Qualifications

  • Bachelor's degree in social work, Nursing, Human Services or related field.
  • Current Social Work or Registered Nurse license.
  • Three years of supervisory or managerial experience and a minimum of one year of transition support experience.
  • Excellent verbal and written communication skills.
  • Excellent interpersonal and conflict resolution skills.
  • Excellent organizational skills and attention to detail.
  • Strong analytical and problem-solving skills.
  • Strong supervisory and leadership skills.
  • Proficient with Microsoft Office Suite or related software.

Responsibilities

  • Serves as the primary contact for the Community Transition Liaison Program (CTLP) to AGE.
  • Ensures the quality of the overall administration of the CTLP in accordance with programmatic requirements set forth by AGE.
  • Provides supervision, direction, and oversight to the CTLP Team(s).
  • Ensures timeliness and quality of all CTLP documentation, including consumer and service data.
  • Monitors case records in compliance with regulations, standards and protocols.
  • Contributes to the development of procedures, standards and guidelines for the operation of the team.
  • Collaborates with facility staff, CTLP staff, and other OCES staff as appropriate, to plan and coordinate discharge activities.
  • Maintains a detailed knowledge of current OCES policies and standards, and funding source regulations.
  • Other duties as assigned.
