Job summary

ReDiscover is seeking a Community Support Specialist to work with adults and youth, providing case management and crisis support within a mission-driven non-profit. You will collaborate with a multidisciplinary team to connect clients to long-term supports, while helping them navigate transportation and access to community programs.

Qualified candidates have a 4-year degree or equivalent experience, strong customer service, and excellent communication skills.

Qualifications

• 4 year degree or 4 years of equivalent experience required.
• At least two years of clinical experience in a mental health setting preferred.
• Knowledge of Kansas City metro area resources and health/social service systems preferred.
• Excellent customer service with a recovery focus.
• Strong written, oral, and interpersonal communication skills.

Responsibilities

• Maintain and interact with a caseload of clients, in-person or over the phone.
• Facilitate long-range outcomes: symptom management, employment, independent living.
• Engage hard-to-serve clients by investigating resources to gain access.
• Help remove barriers and refer clients to educational or work programs.
• Assist with transportation barriers and transport clients as needed.
• Collect and document client information in the electronic medical record.
• Collaborate with an internal multidisciplinary team to coordinate care.

This program works to connect clients to long term supports while assisting through their immediate crisis and is often the first point for services for people in need.What You’ll DoMaintain and interact with a caseload of clients with diverse characteristics either in-person or over the phone.Facilitate each client’s achievement of long range outcomes of symptom management, employment and independent living based upon the needs of the client.Engage difficult to serve clients by investigating all potential resources in order to gain access and make connections.Help remove barriers for clients, which may include helping clients participate in community or social service programs and referring clients to educational or working programs.Assist clients with transportation barriers as identified in their treatment plan.
